Please make sure your surgery authorization/pre-certification is on file prior to surgery.
Prior to surgery, we inform all patients of financial obligations, so there are no surprises. We will inform you by text/email prior to surgery about the amount due on the day of surgery.
On the day of surgery, we will collect any co-payments, unmet deductibles, and co-insurance amounts.
Please note:
The co-insurance amounts collected are an estimate only. Your total financial obligation will be determined by your insurance company which will appear on the explanation of benefits.
If our estimated amount is less than the actual amount, we will bill you for the difference.
Example: Estimated amount is $300, but the actual amount is $400, then we will bill you $100.
If our estimated amount is more, we will refund you the difference.
Example: Estimated amount is $300, but the actual amount is $200, then we will refund you $100.
We encourage you to also contact your surgeon, anesthesia group and your insurance company if you have any questions about your financial obligations. As a reminder, you are still responsible for any balance left by your insurance(s). If you have any questions or concerns, please call our office at (408) 347-1875.
If you do not have insurance, you are responsible for all surgeon, facility, anesthesia & pathology lab fees, and will need to arrange the payments with each individual entities (surgeon, facility, anesthesia and lab) separately. We will not accept any payments on behalf of another entities.

Checks should be made payables to Montpelier Surgery Center. We accept Cash, Check, Visa, Mastercard, American Express and Discover card.
